What causes deformities in chicks?

So far out of 3 chicks from the same hen, two were born with
deformities and died 12 hrs later! the third chick needed help
hatching, no deformities is healthy chirping and attempting to run
around, it has already tryed to eat the wood chips in the brooder,
lol! But i am worried about it too! It sleeps alot, very chirpy
especially when i talk! it is alone as none of the others survived
and the rest of the hatch has not even pipped yet! I was able to
gently wash the dryed sticky off with some warm water, and very
mild baby soap, it has fluffed out with nicely eyes open, cheeping
like the dickens, and trying to walk but more like stumbling!
Any ideas as to what is causing the deformities, the incubator
has been at a constant 99.5 and 50% humidity, except for lockdown
when i increased the humidity to 70%! I hadn't opened the
incubator till after the two died and the other one needed help!
No others of the 6 have pipped or hatched yet!
Could the hen and roo be related or could one of them have a gene
issue!

What deformities did you see Mommam hen? If one chick hatched, they
all had the same chance so its more likely to be health of the
parents or genetics.
(I would suggest plain kitchen roll for the first week too. It will
eat woodchips and it can cause them problems)
